JCS is dedicated to helping aging individuals and their families navigate life transitions and changing circumstances. Our professional staff offer services and resources that enable older adults to maintain independent lives in the community for as long as possible and provide peace of mind to their families.

Case Management
Case Management starts with a comprehensive evaluation of an older person’s wellness, social and financial needs. Our case managers provide assistance in accessing need-based governmental programs, such as transportation, utility assistance, food programs and supports for living at home, and also connect eligible individuals to JCS Emergency Assistance and Holocaust Survivor services for which they are eligible.
Eldercare Management
Eldercare Management, a fee for service program, supports aging individuals in their desire to continue living in their own homes or other settings with maximum independence and dignity, providing them, their family and caregivers with peace of mind.

Community Programs and Support Groups

Patient Care Connection
This innovative program is designed to help older adults and adults with disabilities, and their respective caregivers, overcome challenges that may be affecting their health, well-being, and ability to age safely in their own homes. Dedicated JCS social workers partner with local medical practices to address the psychosocial and economic factors impacting the patient’s wellbeing and ability to achieve or maintain optimal health.
