JCS is dedicated to helping aging individuals and their families navigate life transitions and changing circumstances. Our professional staff offer services and resources that enable older adults to maintain independent lives in the community for as long as possible and provide peace of mind to their families. 

Service Coordination

Service Coordination starts with a comprehensive evaluation of an older adult’s wellness, social and financial needs. Our case managers help the older adult access need-based governmental programs, such as transportation, utility assistance, food programs and supports for living at home, and also connect eligible individuals to JCS Emergency Assistance and Holocaust Survivor services  for which they are eligible.
 
Case managers continue providing support and assessments that enable older adults to age in place safely. 

 

Eldercare Management

Eldercare  Management,  a fee for service program,  supports aging individuals in their desire to continue living in their own homes or other settings with maximum independence and dignity while providing their family and caregivers with peace of mind.

Community Programs and Support Groups

We offer a variety of  informational programs and support groups to help people of all ages navigate the changes, challenges, and transitions they encounter throughout the lifecycle.  

Patient Care Connection

This innovative program is designed to help older adults and adults with disabilities, and their respective caregivers, overcome challenges that may be affecting their health, well-being, and ability to age safely in their own homes. Dedicated JCS social workers partner with local medical practices to address the psychosocial and economic factors impacting the patient’s wellbeing and ability to achieve or maintain optimal health.
